Transitional Care

After discharge in an Inpatient stay, we see the patient within 7-14 days which is considered the transitional period. Managing the patient medical needs, typically 30-day management cycle. The primary goal is to ensure that the patient doesn’t need to be readmitted to the hospital unnecessarily. We see patients sooner than their PCP.
